Head of Bed after Ischemic stroke Thrombectomy (HoBIT) is an adaptive, prospective, open‑label, blinded‑endpoint (PROBE) study evaluating whether a 0‑degree head‑of‑bed (HoB) position improves functional outcomes after endovascular thrombectomy (EVT) for acute ischemic stroke due to large‑vessel occlusion. The trial compares flat positioning to ≥30 degrees and measures functional status at 90 days as the primary outcome, along with early neurological changes, quality of life, and safety events. Adults undergoing EVT for anterior‑circulation large‑vessel occlusion are eligible, with specific exclusions to ensure safe and feasible positioning. An interim analysis after the first 750 participants will guide potential adaptations to enrollment criteria and sample size based on prespecified thresholds and DSMB recommendations.
